First and foremost, the state of NH requires current immunizations and physicals that include vision and hearing, BEFORE the first day of school. I will accept 4 year physicals to start the school year as long as I have a date for their 5 year physical appointment
If your student has an allergy or asthma, I require an allergy or asthma action plan along with their epipen/inhaler. These action plans are my medical directive for administering these medications.
If your student is on a daily medication, please let me know what the medication is and the dosage so that I may better take care of your student when they come to my office. If these medications are changed or your student starts a new one, please keep me in the loop. If you would like your student to take daily medications at school, I have a form or you may have your child's PCP send in a note. DO NOT SEND MEDICATION OF ANY KIND IN WITH YOUR STUDENT! This poses a risk to other students. Medications must be hand delivered to me or the front office.
My goal is to keep your student in class and learning. Every minute they are in my office, late to school or absent affects your child’s learning. I use a computer program called SNAP for all things medical. This is where you give permission for over-the-counter medications, and immunizations, physicals and screenings are logged.
Please be on the lookout for an email from SNAP that states “Grantham Village School welcomes you to the SNAP Health Portal”. Please open the email and follow the directions. SNAP also allows you to upload physicals, immunizations, screenings, or PCP notes. I have a tutorial on my website. Also, if you give your student an over the counter medication in the morning, please let me know the time and medication so that I don’t give another dose too soon.
Keep your child home if they have:
Fever over 100
Diarrhea or vomiting within 24 hours
Undetermined rash
Red itchy eyes with sticky, goopy drainage
Scabby sores around their mouth/nose/face
Lice before treatment
If your child is sick, please enter it into PickUp Patrol before 9 am. You can find this information on the website.
I generally email parents if I see your child at the end of the day. I will call if I have a concern, a question or they need to go home.
