Documents Required for Registration:
- Birth Certificate
- Residency Affidavit (Fillable PDF)
- Home Language Survey (Fillable PDF)
- Student Record Release Form
- Kindergarten Questionnaire
- Most recent physical and immunization records
- 2 Proof of Residency Documentation
- Copy of utility bill with physical address listed AND one of the following:

Copy of lease agreement with the term/dates listed. The agreement MUST contain property owners name, address, phone number, and signature; and name and signature of parent/guardian.

***Note: Moving in with a person in an apartment will not be accepted, unless your name is on the lease or listed on the lease under occupants. Also, if you are living in a shared housing situation with a renter or property owner, and your name isn't on legal documents, you will need to complete and have notarized a "Cohabitation Affidavit". Please contact the Front Office for that form.***

GVS Before School Care
GVS Before School Program operates from 7:30 - 8:10am, Monday through Friday when school is in session. It is for students in grades K-6th. Students who attend will be in the cafeteria with Mrs. Googins and Mrs. Bailey. Registration is required, and a fee of $3.00/day will be billed for days attended. Free or reduced status may impact the cost.
Morning Recess operates 8:10 - 8:30am, Monday through Friday when school is in session. Morning recess is available at no charge to any student in grades K-6, and no registration is necessary.
The school day begins promptly at 8:30am.
